A. Profile component
1. Application for food hygiene and safety certificate enclosed.
2. A notarized copy of the business registration certificate.
3. Explanation of material facilities, equipment and tools to ensure hygiene and food safety (by the company based on the actual situation to prepare - with the enclosed form). including:
- Drawings of the plan of production and business premises and surrounding areas.
- Description of the processing (technological process) for the product group or each specific product.
4. A notarized copy of the "certificate of eligibility for health" issued by the establishment owner and the person directly engaged in food production and trading. Include :
- A copy of the certificate of eligibility for health and stool examination by the employer and the person directly (certified by the establishment);
- A copy of the certificate of food safety knowledge (certified by the establishment).
5. A notarized copy of the certificate of training in knowledge on food hygiene and safety by the establishment owner and the person directly engaged in food production and trading (after compiling the dossier, Minh Law Firm Khue will prepare documents for the company to send the trainer, based on the situation and demand from the company.
B. Competent state agencies
Department of Food Hygiene and Safety under Hanoi Department of Health
Time of evaluation: 15 days.
After the dossier is evaluated, the inspection team will directly inspect the production facilities of the company and conduct the assessment:
- Having met the standard, the certificate will be issued after 7 days
- Failure: The company needs to complete the required follow-up evaluation.
